What is the North Renfrew Family Health Team? The present team of doctors, nurse practitioners, registered practical nurses, and office administration, along with a social worker, dietitian and pharmacist, provide access to primary medical care to local residents registered with the team. Telemedicine services for specialist consultations are available as needed. The North Renfrew Family Health Team, located in the medical building attached to the Hospital, opened its doors in 2006. Since then, it has provided services to residents of the Towns of Deep River and Laurentian Hills and the Township of Head, Clara and Maria. What is the patient experience within a Family Health Team? Family Health Teams are patient centred and patients will have access to a number of primary health care services from a variety of team members, including a family doctor. The team approach means service is relatively easy to access on a timely basis. Patients will be followed by a doctor or nurse practitioner, but depending on their clinical needs, they may be referred to other health care professionals within the Family Health Team. Patients may also self-refer to the social worker and/or pharmacist. The team approach often allows for more time and discussion of health concerns than might be possible in a more traditional medical office setting. Also, patients will have access to additional services such as hypertension management programs and smoking cessation counseling
