We are committed to protecting the privacy and confidentiality of your personal health information. We collect, use, and share your information only as necessary to provide care, support health system planning, meet legal obligations, and improve our services.

Your Rights & Our Responsibilities

Ontario law gives you important rights when it comes to your personal health information.

You have the right to:

  • Access and request a copy of your health record
  • Request corrections to your personal health information
  • Place limits on how your information is shared with others
  • Be notified if your information is lost, stolen, or accessed improperly

Deep River & District Health is responsible for:

  • Collecting only the information needed to provide your care
  • Keeping your health information private, secure, and accurate
  • Using or sharing your information only with your consent or as permitted by law
  • Responding to any concerns you have about your personal health information

Using & Sharing Your Information

We may use or share your personal health information to:

  • Provide care within and beyond our organization
  • Communicate with your healthcare team
  • Obtain payment from OHIP or your insurance provider
  • Support research, quality improvement, and system planning
  • Meet legal and regulatory obligations

Unless you request otherwise, we may share limited information, such as your location or general condition, with family, friends, or spiritual care providers.

In some cases, we may also share limited contact information with the Deep River & District Health Foundation for fundraising purposes. You may opt out at any time.

Accessing Your Health Information with MyChart

MyChart is a secure online tool that allows you to view and manage your health information anytime, anywhere. With MyChart, you can:

  • Access recent test results, medications, and clinical notes
  • View upcoming appointments and visit history
  • Download portions of your health record

Accessing Your Health Record

If you need information that is not available in MyChart or prefer to request a copy by mail or fax, please submit a written request to:

Your request must include your name, contact details, and the details of the information you are requesting.

Requesting Information for Others

To request records on behalf of someone else, such as a child, deceased relative, or person under your legal care, you will need to provide the following:

  • Proof of your legal authority (e.g., Substitute Decision Maker, Power of Attorney, legal guardian, Executor of their estate)
  • A written request with your contact details and the information you are requesting

Sharing with Other Providers or Third Parties

We may share your information with other healthcare providers involved in your care, such as family doctors or community support agencies. You may also request to have your record sent to a lawyer, insurer, or other third party by submitting written consent.

Correcting Your Record

To request a correction to your health record, you can contact the Health Records Department at 613-584-3333 ext. 7150 or submit a correction request through MyChart.
