We welcome all compliments, comments, suggestions, and concerns. Hearing from you helps us enhance the experience for patients, residents, families, and visitors across our organization.

Patient Experience Surveys

Excellent, compassionate care starts with listening. Our digital surveys give patients a simple way to tell us how we’re doing.

Through a partnership with Qualtrics, we offer a secure, user-friendly survey tool that allows patients to share their experience confidentially and anonymously. Patients who visit our hospital’s Emergency Department or are discharged from the Inpatient Unit receive an email invitation to complete the survey after their visit. It can be filled out anytime, from anywhere, in just a few minutes.

Each response helps us better understand what’s working well and where we can improve. We’re grateful to all who take the time to share their feedback.

Frequently Asked Questions (FAQ)

Why does DRDH collect patients’ email addresses?

We collect email addresses to send patients a patient experience survey following their visit or admission. Email addresses may also be collected for those who choose to sign up for MyChart, our secure online patient portal.

If I don’t have an email address, can my partner, parent, or family member provide their email address instead?

Yes. With your verbal consent to the Registration Clerk, a family member or caregiver may provide their email address on your behalf. Please note, the survey is intended for you, the patient, to complete.

When will I receive the survey?

Surveys are typically sent by email one to two days after your visit or discharge.

When I complete the survey, will the hospital staff know who the feedback is from?

No. Your responses are confidential. They are combined with other patient responses in a report shared with the hospital in aggregate form. Your identity will not be linked to your feedback.

How are my responses used? Who has access to these responses?

Your feedback helps us better understand the care you received and identify opportunities to improve. Survey results will help inform and improve person-centred care across the hospital as well as the delivery of services. These surveys are confidential, and your personal information will not be shared.

Will I receive multiple surveys if I visit DRDH more than once?

No. After you complete a survey, your name is removed from the invitation list to prevent receiving multiple surveys within a three-month period.

Who will the survey come from?

The survey will be sent by email from Deep River & District Health. Please check your inbox (and junk folder) for a message with this sender name.

Will a patient’s email address be shared with a third party?

No. Patient contact information is only shared between Deep River & District Health and Qualtrics, the trusted third-party provider administering the survey on our behalf. Both organizations fully comply with federal and provincial privacy laws governing the protection of personal health information.
