Patients rostered to the North Renfrew Family Health Team have access to a wide range of primary care services designed to support their health at every stage of life.

Our team-based approach ensures coordinated, comprehensive care tailored to your individual needs.

After Hours Urgent Care Clinics

Our after hours clinics provide access to same-day care for urgent, non-emergency concerns. Appointments are available to current patients only between 5:00 p.m. – 8:00 p.m. on select evenings.

These clinics are not walk-in. Please call the office on the day you wish to be seen to book an appointment.

Hypertension Management

We offer 24-hour blood pressure monitoring at no cost, along with personalized follow-up and support from our interdisciplinary healthcare team. This service helps patients better understand and manage high blood pressure as part of their overall care plan.

Smoking Cessation

Patients can self-refer to our smoking cessation program, which is offered in partnership with the Ottawa Model for Smoking Cessation. This service provides free resources and personalized support to help you quit or reduce smoking or vaping, at your own pace.

Nutrition Counselling

Patients can self-refer to work one-on-one with our Registered Dietitian for management of a variety of health concerns, including high blood pressure, pre-diabetes, IBS, food sensitivities, allergies, weight management, picky eating, eating disorders, and more.

Social Work Counselling

Our Social Worker provides individual counselling for both adults and children who are rostered patients of the Family Health Team, offering support for mental health, stress, life transitions, and other personal challenges.

Preventative Screening

We support your long-term health through regular screening and immunization services. This includes cervical and colorectal cancer screening, flu shots, childhood vaccinations such as well-baby immunizations, and more.

Telemedicine

We offer access to secure virtual specialist appointments through the Ontario Telemedicine Network (OTN). This service allows patients to connect with specialists located outside our region, reducing the need for long travel times. Telemedicine appointments are held in a private room at the Family Health Team and are supported by our trained staff.

This service is available by referral only and can help patients access timely care for a range of health concerns, including follow-up visits, specialist assessments, and chronic disease management.
