Admissions to all long-term care homes in Ontario, including Four Seasons Lodge long-term care home, are coordinated through Ontario Health atHome.
Once you or your loved one is ready to begin the process, Ontario Health atHome will guide you every step of the way. Their team can assist with eligibility assessments, help you explore and select homes that meet your needs, support you through the application process, provide information about wait times, and help prepare for the move.
