Duties: - Assists in management of patient care plans, coordinates patient discharge planning, and communication between physicians and interdisciplinary team internally and externally.
- Provides clinical expertise and advice either directly to patients or to staff.
- Provides leadership by identifying gaps in practice, communicates and collaborates with clinical teams and leadership to incorporate best practices that promote quality patient care.
- Coordinates the nursing team and is involved with and supportive of mentoring, prioritizing tasks and monitoring quality of work.
- Provides assistance with Long-term Care home staff.
- Practice as a member of the multidisciplinary team to provide quality nursing care and education for patients and their families.
- Assist with planning, development, coordinates implementation and evaluation of clinical staff education activities.
- Supports nursing mentorship programs.
- Adheres to and is responsible for applying all policies and procedures.
- Supports and demonstrates the mission, vision and values of Organization.
- Other duties may be assigned and may not be limited to the scope of an individual department.
Qualifications: - Current Registration with the College of Nurses of Ontario as a Registered Nurse, and be a member in good standing required.
- Bachelor of Science in Nursing or equivalent education and relevant experience.
- Current Basic Life Support (BLS) certification
- Current Advanced Cardiac Life Support (ACLS) certification
- 12 Lead ECG Interpretation Course or equivalent
- CTAS certification
- TNCC and PALS certification preferred
- Emergency Nursing Certificate, or Critical Care Certificate or CNA Certification inEmergency or Critical Care Nursing preferred
- Gentle Persuasive Approach certificate preferred
- Current member in related professional nursing, interest group, or association preferred
- Minimum of 3-5 years recent clinical experience in related progressive positions preferred.
- Recent clinical experience in an acute care setting.
- Experience leading interdisciplinary teams.
- Dedication to Person-Centered Care.
- Knowledge of clinical care best practices, and community resources.
- Proven ability to supervise, coach and train with an understanding of adult learning principles.
- Knowledge of the accreditation, emergency planning processes, project management and quality improvement principles.
- Demonstrated critical thinking and problem solving abilities required.
- Exemplary interpersonal, communication and organizational skills required.
- Excellent attendance record required.
- Must agree to adhere to DRDH standards of behaviour.
