Position Summary

Practices as a member of the multidisciplinary team to provide direct care to support patients to meet their physical, emotional, social, cultural and spiritual needs as outlined by the organization’s policies and procedures.

 

Position Relationships

Under the direction of the Manager of Clinical Services – Acute Care, works in collaboration with the Interdisciplinary Team and within established reporting structures. Provides care under the direction of the designated Registered Nursing Staff.


Duties: 

  • Assists patients with activities of daily living, including personal care, using the principles of Person focused care.
  • Contributes to the quality of life of patients by meeting social, emotional, spiritual and physical needs and promoting independence, dignity, mobility, personal appearance, comfort, and safety.
  • Works as a member of the team, communicates effectively, and observes and reports changes in patient condition to the Registered Practical Nurse or Registered Nurse.
  • Adheres to all policies and procedures.
  • Supports and demonstrates the mission, vision and values of the organization.
  • Other duties may be assigned and may not be limited to the scope of an individual department.

 

Qualifications:

  • Personal support worker certificate, completion of first year nursing program, or equivalent as per O. Reg. 246/22 of the Fixing Long-Term Care Act, 2021.
  • Current Basic Cardiac Life Support (BCLS) certification.
  • Gentle Persuasive Approach certificate preferred.
  • Education and training related to dementia and geriatric care preferred: included but not limited to Montessori, restorative care approaches, palliative care, etc.
  • Experience in a long-term care setting preferred.
  • Experience in hospital setting is an asset.
  • Therapeutic verbal and written communication skills required.
  • Ability to work well with others in a team approach, and adapt to changing situations required.
  • Computer literacy required.
  • Exceptional Attendance required.
  • Must agree to adhere to the standards of behaviour of the organization.
