DRDH Seeking Feedback from Patients - COVID-19 Debrief Survey

In response to the COVID-19 pandemic in early 2020, the Deep River and District Hospital, the Four Seasons Lodge Long-Term Care, and the North Renfrew Family Health Team activated the organization’s Emergency Operations Center. Since that time, a large part of our operations has been focused on pandemic response, with the goal to ensure safety for our staff / physicians as well as maintain health services and safety for our patients, residents and community.

With increasing vaccination rates and decreasing case counts, we are now starting to look ahead to a gradual reopening of both the organization and the province. As part of this reopening, we will be conducting a debriefing exercise to identify learnings from our organization’s emergency response during the pandemic. A similar debriefing exercise and survey were conducted in August 2020, which helped inform decision making during the second and third waves of the COVID-19 pandemic.

As part of enhancing our capacity to respond to future emergencies, we are inviting those who have received care at the Hospital, Family Health Team, COVID-19 testing centre, or COVID-19 vaccine clinic over the last 6 months to provide feedback by completing the survey below.

Your feedback will be used to identify successes, and areas where we can improve. Our aim is to use your input to identify how we can continue to work together to build a strong, resilient organization and community, and be prepared for future emergencies.

Your time to provide input and share your experience through completing the survey is greatly appreciated. We ask that surveys are completed as soon as possible, no later than Wednesday, June 16, 2021.
